Did you run 'makeinfo --help'?  Did you read the output?

Perhaps the naming is unfortunate, but the page does describe how to
report any problem with Cygwin, not just bugs.

One discrepancy right here: you don't run makeinfo on .info files, you run
it on .texi files (which, if you had read the output of 'makeinfo --help',
you'd know already).  FYI, I don't get any reports of the missing TOC.

Not that it's very relevant anymore, but you should have included this
file as an attachment, as your mailer surely screwed up the non-printable
characters in that file.  Just a hint for the future.
